Job Skills/Requirements
Responsible for maintaining ongoing tracking and appropriate documentation of referrals ordered by the medical provider. Providing excellent patient services following the patient centered model of care adopted by Park DuValle CHC.

1.Work with Medicaid/Medicare/Commercial Ins., discuss patient payment options.

2.Ensure complete and accurate registration , including patient demographic and current insurance information.

3.Work with the Allscripts Practice Management system.

4.Assemble information concerning patient’s clinical background and referral needs. Per referral guidelines, provide appropriate clinical information to specialist.

5.Speak with patients by telephone in order to successfully resolve questions and concerns.

6.Contacts review organizations and insurance companies to ensure prior approval requirements are met. Present necessary medical information such as history, diagnosis and prognosis.

7.Maintain confidentiality of patient information in accordance with HIPPA law.

8.Demonstrate initiative and self-management skills in fulfilling job duties.

9.Answers questions from patients, clerical staff and insurance companies.

10.Review details and expectation about the referral with patients.

11.Assist patients in problem solving potential issues related to the health care system, financial or social barriers.

12.May assume advocate role on the patient’s behalf to ensure necessary supplies/services for the patient in a timely manner.

13.Remind patients of scheduled appointments via mail or phone.

14.Participates in educational activities and attends monthly staff meetings.

15.Conducts self in accordance with PDCHCs employee manual.
